Pet Parent Registration

Welcome to the free pet parent registration site. To sign up for the service, you will need to read through the Teleconsulting Informed Consent paragraph below and sign your name. You will also be asked for your personal details and the details of your veterinarian before submission. Once you have submitted this form, we will contact your veterinarian and register their practice if it is not already in our system. You will be contacted within 3 business days to confirm that the registration process is complete and you will then be able to request an appointment.

Teleconsulting Informed Consent

Teleconsulting provides a useful and convenient way for clients and their pets to receive veterinary care. As with any veterinary visit, there are certain benefits and risks associated with teleconsulting that you need to understand.

Teleconsulting allows clients access to specialists that they might not be able to see otherwise, it reduces wait times for appointment scheduling, and provides an additional level of care beyond your local veterinarian. Although it is an alternative to traditional, in-person veterinary care, teleconsulting is limited by the information that the veterinarian is provided because he or she is not able to do an in-person physical exam and may not have all the information about your pet or your pet’s condition. It may also be limited by other factors outside the veterinarian’s control, such as incomplete or inaccurate data provided by you or the distortions of diagnostic images or specimen due to electronic transmission issues.

By signing below, you are agreeing to the following:

  1. You acknowledge that you understand the risks associated with teleconsulting, including that it may not be a complete solution for your pet and that you may need to seek additional, in-person care for your pet.

  2. You understand that an in-person physical exam and consultation provides the best setting for pet care, but you accept the risks and limitations associated with the teleconsulting format (e.g., video, telephone, email).

  3. You want to proceed with this consultation as a supplement to your pet’s ongoing care with a primary veterinarian.
